I've never really understood what anti-aliasing does exactly, but I've noticed that it's almost always turned off in default graphics options in video games. I never use it but I've wondered if anyone here uses it and if it makes a significant difference to the visual quality?

It does make some games look much better, especially games with lots of small details like foliage (I'm thinking of Rise of the Tomb Raider right now). Without antialiasing, the image looks "noisy" in scenes with too much stuff.
Still, antialiasing is the first setting I will turn off if the game isn't hitting 60fps - so, while it does make the game look better, it's not that important.
